Rowan's Law Acknowledgement
All parents/guardians must review the OWHA Rowan's Law Concussion Awareness Resource with their child and acknowledge they have done so. All coaches, trainers, managers, on-ice helpers and players age 18 or older must review OWHA Rowan's Law Concussion Awareness Resource and acknowledge they have done so.

Participant Information

If you have more than one daughter registered, you will need to complete this form for each daughter. For team staff, you must complete this for yourself even if you have completed it for your daughter. ​

The Ontario Government has adopted the Statute “Rowan’s Law” that includes mandatory requirements for all sport, including the Ontario Women’s Hockey Association (OWHA). The OWHA’s Concussion Awareness Resource is available through the link below and includes Code of Conduct, Removal from Play, Return to Play, age-appropriate material and links to valuable additional resources.
 
The OWHA Concussion Awareness Resource and Code of Conduct must be read on an annual basis before an individual registers/participates with the OWHA or with an OWHA member..
